Teresian House is currently hiring a full-time Admissions Screener.
Are you a compassionate and motivated professional seeking a rewarding career opportunity? Teresian House is a 300 bed skilled nursing facility. The Admissions Screener performs a broad range of admissions functions including data entry, phone calls, processing admission applications and obtaining authorizations. The screener is also responsible for establishing and maintaining communications between Teresian House and referring publics, consumers, physicians, and third party payers; providing maximum flexibility in coordinating the payer's needs with patient outcomes; communicating effectively with the treatment team, and influencing the treatment approach when needed; and achieving census targets by working in collaboration with facility leadership.
The Admissions Screener is responsible for oversight of all aspects of the assessment process of a patient's admission into sub-acute rehabilitation, including conducting preliminary screening of admissions candidates, visiting patients, educating interested parties about the services offered, and deciding whether they are suitable for admission to Teresian House.
Requirements:
- BSW or MSW preferred.
- Long-term care, acute care, sub-acute or hospital care experience preferred
- Excellent verbal and written communication skills.
- Proficiency with computers.
- Travel to area hospitals to screen medical records for potential residents.
