Job Summary

We are searching for a HIM Document Specialist II-- someone who works well in a fast-paced setting. In this position, you will Works independently to provide coverage for a variety of tasks associated with a remote records room(s). Routine Medical Record Department technical processing functions include: record retrieval and delivery, chart pick up, record processing for completion, assembly, analysis, transcription processing, physician assistance, microfilm processing, the release of information, chart locator updates, chart release, chart deficiency, telephone coverage utilizing appropriate customer service skills, and filing.

Responsibilities

Job Duties & Responsibilities

  • Process routine disclosures of Protected Health Information (PHI) to patients, families, insurance companies, attorneys, etc. according to federal/state laws and regulations and non-routine disclosure of PHI in response to court orders and subpoenas.
  • Retrieves, delivers, compiles, scans, and processes medical records.
  • Analyzes medical records for accuracy. Analyzes medical records for deficiency with JCAHO standards, medical staff by-laws, rules, and regulations for accuracy. Follows the department analysis guidelines. Interprets notes to determine whether there are deficiencies in medical records. Determines appropriate physicians to complete deficiencies in the medical records. Assist others in resolving problem charts. Enters deficiencies into the Electronic Health Records.
  • Processes STAT requests, walk-ins, and transfers.
  • Provides excellent customer service to patients, physicians, staff, and all other customers.

Qualifications

Skills & Requirements

  • Being fully vaccinated against COVID-19, including any booster dose(s) of the COVID-19 vaccine recommended by the Centers for Disease Control when eligible, is required for all employees at Texas Children’s unless approved for a medical or religious exemption.
  • High School Diploma or GED
  • 2 years’ experience working with electronic records
