FUNCTION
Residential Treatment Model (RTM) Coordinator is responsible for creating and implementing AFC’s first trauma-informed, evidenced-based model of clinical support across housing sites. The RTM Coordinator will manage a team of mental health professionals, lead treatment planning meetings and faciliate clinical training for housing staff. The RTM Coordinator is a Licensed Clinical Social Worker role, who will be supervised by the Senior Director of Housing Programs, with additional clinical supervision by the Senior Director of Medical and Mental Health Services.
TOP RESPONSIBILITIES
The Residential Treatment Model Coordinator is responsible for the following:
Manage RTM Social Work team, including clinical supervision
Comprehensive evaluation to assess emotional, behavioral, medical, educational, and social needs for runaway and homeless LGBTQ youth
Develop a program design for clinical services and support in housing sites
Create and facilitate ongoing clinical training for housing staff
Provide crisis intervention, Individual and group therapy, as needed
Offer clinical guidance and support to frontline staff
Coordinate with care teams including case managers, therapists, clinical coordinator and psychiatrist
Progress note documentation and data entry in required databases
Participate in weekly treatment planning meeting, case conferencing, housing staff meeting, training and supervision
Create and maintain systems of data collection and management
EDUCATION REQUIREMENTS
[X] Masters Degree
LMSW (NYS license required) , LCSW within 8 months of hire, LCSW preferred

TECHNICAL SKILL REQUIREMENTS
Knowledgeable and comfortability in utilizing DBT skills , Skilled in Trauma Informed Care, Harm Reduction & substance use intervention, LGBTQ Competency, TGNCNB Competency
PREFERRED QUALITIES
Must be knowledgeable in therapeutic engagement and trauma services. Experience working with adolescent/young adult population required. Knowledge of psychosocial needs of LGBTQ/homeless population is helpful.
