By leading the way in global research, providing assistance for people with ALS through a nationwide network of chapters, coordinating multidisciplinary care through certified clinical care centers, and fostering government partnerships. Care Services Coordinators will provide comprehensive and professional services, community resources, and disease management to people with ALS, their families, and caregivers within the scope of their catchment area.
*This position is currently a hybrid position and will have remote work in clinic or direct services, as well as occasional travel to our offices in Richmond and/or metropolitan D.C area. This position serves our Hampton Roads, Virginia territory area.

ESSENTIAL DUTIES AND RESPONSIBILITIES.
- Grow and expand knowledge of ALS and awareness of The ALS Association.
- Identify and develop local and regional resources including healthcare providers, agencies, companies, and community partners.
- Build and maintain relationships that help people living with ALS access care according to evidence-based best practices.
- Assess both physical and psychosocial needs of ALS patients and families, based on knowledge of ALS, the disease, and its ramifications.
- Provide health education and outreach to patients, caregivers, families, and community healthcare professionals.
- Possess a strong positive work ethic that embraces a culture of teamwork.
CARE SERVICES
- Conduct specialized care consultations and needs assessments for people living with ALS, caregivers, and families in a variety of settings including office visits, by phone and email, in clinic, at resource support groups or at a home visit, when appropriate.
- Plan and schedule so that services are delivered as effectively and efficiently as possible to meet the current Chapter goal of at least one significant contact each quarter, as deemed appropriate, with each person living with ALS.
- Respond to individuals with ALS, their families, and caregivers no later than 24 hours of receiving a call or email.
- Act as a patient advocate to help people living with ALS, caregivers, and families obtain equipment, services, and care to improve quality of life.
- Connect people living with ALS, caregivers, and families to Chapter Care Services programs and community resources.
- Recruit and train volunteers for patients and families.
- Educate and inform the general public and professionals about the nature of ALS, its management, related research, and the existence of services of The ALS Association.
ADMINISTRATION
- Update the Director, Care Services with respect to local community resources and educate patients and families regarding available resources.
- Attend quarterly Care Services staff meetings at the Chapter’s Administrative Office, Virginia Regional Office, and other locations throughout the territory.
- Identify walk team or donor prospects to benefit the Chapter fundraising program.
PROGRAM MANAGEMENT
- Facilitate resource support group meetings, engage participants, and schedule speakers as appropriate to meet the needs of the group.
- Serve as a member of the ALS Clinic multidisciplinary team and function as the liaison between the Clinic and the Chapter.
- Use established policies and procedures, guidelines, and records case notes in a timely manner.
- Participate in Care Services program measurement and evaluation.
- Contribute to the multi-state Care Services team and work with other Chapter staff on additional projects as directed.
- Other duties as assigned.
QUALIFICATIONS:
- Bachelor’s degree in social work, nursing, public health or related area, Master’s preferred. Occupational therapist, physical therapist, speech therapist or direct experience working with the ALS population is a possible consideration.
- Ability to work independently with little supervision; must be a self-starter.
- Knowledge of ALS is a plus, but not required.
- Knowledge of Medicare, Medicaid, patient assessments skills, planning and case management preferred.
- Commitment to organization and highly efficient and conscientious about thorough follow-up.
- Skilled communicator, with superior interpersonal, verbal, and written skills.
- Able to handle confidential information and maintain high confidentiality.
- Excellent organizational and writing skills as well as diligent attention to detail.
- Employee must be able to lift or move up to 25 pounds.
- Excellent computer skills. Proficiency with Microsoft Office products and Raisers Edge or other databases
- Must possess valid driver’s license. Required to travel.
- Ability to work night and/or some weekends as required. Must be willing to travel throughout the DC/MD/VA area.
All Care Services Coordinators/Social Workers are expected to be up-to-date on aspects of ALS and it management, coping skills, issues of death and dying and bereavement, related drug trials and research, information on Social Security, Medicare, Medicaid, Veterans’ benefits, and insurance practices and issues that may impact those with ALS.
