Duties and Responsibilities:
Assist individuals with activities of daily living, such as bathing, dressing, grooming, toileting, and eating, according to their individual needs and preferences.
Support individuals in maintaining a clean and safe living environment, including household chores and meal preparation.
Provide transportation and accompany individuals to medical appointments, social outings, recreational activities, and other community-based events.
Assist individuals in developing and practicing life skills, such as budgeting, cooking, shopping, and using public transportation.
Implement individualized support plans and behavior management strategies, ensuring consistency and adherence to best practices.
Foster positive relationships with individuals, families, and other support team members, promoting effective communication and collaboration.
Document individual progress, behaviors, and incidents accurately and timely, in accordance with organizational policies and regulatory requirements.
Monitor and report any changes in individuals' physical or emotional health, and provide appropriate assistance or intervention as needed.
Advocate for individuals' rights, preferences, and choices, while promoting self-advocacy and empowerment.
Participate in training and professional development activities to enhance knowledge and skills related to the field of developmental disabilities and human services.
Plan and implement activities according to the person-centered plan (PCP)
