Job Title: Navigator
General Description:
Responsible and accountable for direct and indirect patient care across the care continuum, with an emphasis on outpatient care, for designated populations. This position is expected to coordinate services and provide targeted consultations, screenings, and support services for patients. The Navigator coordinates a triaged plan of care consisting of specific interventions and/or services centered on meeting the expressed needs of the patient/family within the context of their identified cultural heritage.
Supervision Received: Assigned manager.
Supervision Exercised: None.
Supervision Essential Job Functions:
- Coordinates and facilitates initial consultation and screenings in order to assess service needs.
- Develops initial and updated plans of care to reflect evolving patient needs
- Is part of a multidisciplinary team which meets regularly and contributes to the development of a comprehensive plan of care based on the patient’s needs.
- Accurately documents all navigator proceedings through appropriate recordkeeping in the electronic health records system (RPMS/EHR).
- Develops and maintains cooperative referral relationships; facilitates and tracks referrals.
- Collaborates and maintains positive working relationships with all services and programs designated by patient’s plan of care to ensure appropriate care.
- Identifies, coordinates, and links patients to tribal and community resources.
- Maintains an up-to-date log for all patient callbacks, including call dates, outcomes/results, next planned contact, and related information needed to accurately track and report statistics of patient contacts.
- Independently, or with assistance of qualified team members, utilizes and teaches problem solving techniques to individuals and/or groups.
- Serves as the system navigator for divisions’ shared patients/families and is directly accessible as the point of contact to address their questions and concerns.
- Assists patients/families in resolving potential issues related to health care and financial or social barriers (e.g., request interpreters, transportation services, or prescription assistance as appropriate).
- Provides immediate crisis diversion.
- Attends both mandatory in-service trainings and related conferences and shares content of educational programs attended with team.
- Recovery Resource Services program only: Navigator is responsible for coordinating all available and necessary services for drug court participants including, but not limited to, monitoring each participant’s progress throughout the drug court process, working closely with the court system and law enforcement, attending drug court staffing, and other duties specific to the program.
- Integrated Services program only: Navigator is responsible for coordinating all services a patient receives. This may include, but is not limited to, shared care planning, medication reconciliation, patient support and health education, and screening and tracking outcomes. Individuals in this role should be as much a part of the medical team as other specialty services.
- May engage in public speaking to schools, community organizations, employee groups, etc. to provide standard program information or to serve as a resource on topics approved by the department Secretary
- All other duties as assigned.
Minimum Qualifications
Knowledge, Skills, and Abilities:
Required - Must demonstrate core values consistent with a patient- and family-centered approach to care. Ability to present oneself professionally and exhibit ability to work with various systems while maintaining objectivity and a cooperative attitude. Knowledgeable about drug and alcohol related issues and possess a desire to assist those suffering from them. Excellent time management skills. High level of attention to details and ability to follow through on responsibilities. Ability to establish rapport quickly with patients, families and colleagues. Ability to evaluate situations and adopt an effective course of action; to organize and present facts effectively. Ability to work well under pressure and meet deadlines efficiently and consistently. Must be able to understand oral and written instructions and communicate effectively orally and in writing. Must be familiar with Microsoft Office applications. Must be team oriented, and have initiative to work without supervision. Ability to establish and maintain an effective working relationship with clinical, administrative, and support staff in order to develop professionalism and coordination with the team. Ability to maintain strictest confidentiality guidelines for interactions regarding patient and family information.
Education:
Bachelors Required - Bachelor’s degree is preferred or equivalent experience.
Education Note:
Training And Experience:
Required Experience (0 - 6 months) - Minimum of one year’s work experience in the mental health field or related social services field. Preferred Experience (0 - 6 months) - Experience in a broad range of life experiences preferred.
Licenses, Certifications, Other:
Required - Complete pre-employment drug test with a negative result. Required - Possess a valid driver’s license and a safe driving record. Required - Must possess or obtain appropriate training and or certification in mental health services or related field as may be required to provide adjunctive services (i.e., case management certification, System of Care training, etc.).
Physical Requirements:
Required - Must be able to sit or stand for long periods of time. Requires grasping, standing, sitting, walking, repetitive motions, good balance, listening and hearing ability and visual acuity.
Work Environment:
Work is performed in an office environment and occasionally may work in outside weather conditions. Work may include a certain amount of exposure to unmanaged emotions from patients/family. Work hours are primarily Monday through Friday (8:00 am – 5:00 pm) . Evening and/or weekend work may be necessary.
