Position Title: Nurse Care Mgr II Job Description: Sign On Bonus: $3,000 Remote Based – Virtual, must have hard wired internet access - no WIFI access AND must be within 50 miles or a 1 hour commute to a corporate office. *Must Have Valid, Clinical Licensure in the state of New Jersey or Compact state licensure that covers the state of New Jersey. Work Shift: Monday – Friday: 12 pm - 8:30 pm (Eastern Standard Time). Will also consider Split shift: 8:30 am -12:30 pm / 4:30 pm -8:30 pm (EST) The Nurse Care Manager II is responsible for performing care management within the scope of licensure for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum. Performs duties telephonically. How you will make an impact: Ensures member access to services appropriate to their health needs. Conducts assessments to identify individual needs and a specific care management plan to address objectives and goals as identified during assessment. Implements care plan by facilitating authorizations/referrals as appropriate within benefits structure or through extra-contractual arrangements. Coordinates internal and external resources to meet identified needs. Monitors and evaluates effectiveness of the care management plan and modifies as necessary. Interfaces with Medical Directors and Physician Advisors on the development of care management treatment plans. Assists in problem solving with providers, claims or service issues. Minimum Requirements: Minimum of 3 years of case management experience which may include condition specific clinical experience, home health/discharge planning experience; or any combination of education and experience, which would provide an equivalent background. For internal associates a minimum of 1 year in a Nurse Care Mgr I role. Current, unrestricted RN license in applicable state(s) required. Multi-state licensure is required if this individual is providing services in multiple states. For URAC accredited areas, the following applies: Current and active RN license required in applicable state(s) that allows for an independent assessment to be conducted within their scope of practice. Requires 3 years full-time equivalent of direct clinical care experience to the consumer, 5 years full-time equivalent of direct clinical care experience to the consumer preferred or any combination of education and experience, which would provide an equivalent background, Multi-state licensure is required if this individual is providing services in multiple states. Preferred Skills, Capabilities, and Experiences: AS or BS in nursing preferred. Call Center experience highly preferred. Certification as a Case Manager is preferred. Participation in the American Association of Managed Care Nurses preferred. Prior managed care experience preferred. Knowledge of medical management process and ability to interpret and apply member contracts, member benefits, and managed care products strongly preferred.
