Position Summary:
The Case Management Support Program (CMSP) is designed to provide services to Clients referred by the New York City Human Resources Administration (HRA) that have substance use disorders, are in transitional housing, have a severe mental illness, or other complicating behavioral health issues. Services include CASAC assessment, treatment referrals, and intensive case management (field-based) to meet the complex and immediate needs of the Clients. Applying a client-centered model, Case Managers assist Clients toward the achievement of 1) Referral to substance use and/or mental health treatment; 2) Retention in substance use and/or other mental health treatment programs; 3) Income security by maintaining Case Assistance case continuity, gaining, and retaining employment, or obtaining Federal Disability benefits.
The Individual Placement & Support Specialist of the Case Management Support Program (CMSP) has extensive clinical and organizational knowledge as well as a working network of collaborative linkages with all levels of substance use disorder and behavioral health treatment modalities and recovery-based support system. Develop competitive job opportunities in the community for individuals with mental illness, substance abuse, and/or homelessness using a recovery-oriented principles approach of collaboration and shared decision-making.
Essential Job Functions:
- Facilitate the successful utilization of job readiness training, job retention services, and/or education services.
- Build relationships with potential employers through multiple in-person contacts.
- Engage with community employers, including at least 6 face-to-face employer contacts per week.
- Assist with rapid job searching, resume writing, and teaching interview skills.
- Obtain comprehensive, individualized work incentive planning.
- Provide Clients with accurate information regarding disclosure to the employer.
- Monitor Clients' job retention for 6 months
- Complete vocational assessment and update the vocational profile on a regular basis
- Transport Clients to and from interviews, as needed.
- Complete weekly and monthly performance indicators and record data.
- Act as a liaison between employer and participant, as needed.
- Stress urgency and ownership, guiding Clients through a learn-by-doing process during their job readiness/job search activities.
- Provide vocational counseling, intervention, and program action planning and delivery.
- Document activities, participation, compliance, and employment progress in the Electronic Case Management (ECM) systems.
- Participate in learning collaboratives through both regional and online conferences.
- Perform other related duties as assigned by Supervisor.

Requirements
Education and Experience:
- Bachelor's Degree in a relevant field, such as education, business, vocational rehabilitation, social work, psychology, or occupational rehabilitation from an accredited institution.
- One (1) year of experience in job development, job readiness training, and/or education; or an associate degree in the above relevant fields
- 2 years experience in job development, job readiness, training, and/or education or training; and/or work experience determined to be acceptable to the agency.
- Certified in the Individual Placement and Support (IPS) model of supported employment or willingness to work towards certification within a reasonable timeframe upon hire.
- Bilingual is a plus.
