We are currently seeking a Fee for Service Nurse to join our team providing services in th Bronx and Lower Westchester County area!
The qualified candidate will:
- Provide in-home care management for complex chronically ill clients
- Provide a full range of nursing care duties in the client’s home including assessment, education and medication management
- Coordinate client’s care with care team and create care plan for client’s care
- Document care in EMR in a timely manner
