Summary Description:
Provide work placement and community integration support for Employment Services individuals while in the program, in the community and at community work sites. Contribute to coordination of individual services and communication with community employers, other agency employees, families, government agencies and the community.
Must have a driver's license and a clean driving record, and must own an appropriate vehicle for transporting individuals. Must be able to lift and support the body weight of any program individual with assistance of another employee.
Duties and Responsibilities:
- Monitor and ensure the health, safety, rights and dignity of individuals to encourage their independence while in the program, at community work sites and in the community, in accord with individual choice and human rights.
- Maintain community job placements, monitoring all required work tasks in a manner consistent with employers’ business practices.
- Facilitate natural supports, promote positive peer relations and integration opportunities for long-term supervision and advocacy.
- Complete case management duties for assigned individuals, including advocacy, team planning, and liaison with other agencies and families. In a timely manner, complete all Career Plans, Case Notes, Support Strategies, Assessments, and other required documentation utilized within the program.
- Maintain confidential files, all ISP documentation including data collection, service agreements, treatment plans, attendance, and payroll sheets.
- Plan, participate and implement service planning, attend regular supervision meetings, staff trainings, staff meetings and individual meetings as assigned.
- Use of own dependable vehicle to transport individuals.
- Provide job coaching support as needed including evening and weekend hours.
- Support program participants’ right to express their religious affiliations and cultural traditions through preferred and relevant activities. Support diversity of individuals served through respect of religious preferences and/or cultural backgrounds.
- Participate in planning, development, implementation, and maintenance of off-site employment placements.
- Assist with logistics for off-site placements, including scheduling, transportation, work performance and training.
