Job Duties:
in accordance with established agency policies and procedures; ensures compliance with federal, state, and local
guidelines for all services; assures the quality and growth of the agency through effective promotion and administrative
practices.
1. Directs the coordination and delivery of home health services in regional office, assuring quality of services for clients
a) Strengthens clinical quality and overall efficiency of delivery of home health care services, and assures
compliance with company standards and federal, state, and local guidelines
b) Supervises and is responsible for the provision of home health services delivered by each discipline; coordinates
all patient care services
c) Assigns field staff to incoming referrals, matches abilities and territories with incoming referrals; monitors daily
and weekly personnel schedules to maintain optimum productivity
d) Monitors clinical progress of patient care through weekly care conferences with appropriate caregivers and
medical professionals, evaluation of clinical competency of field staff, multi-disciplinary coordination of client
services, and general patient care management
e) Oversees OASIS assessment data collection for accuracy, completeness, including ICD-9 coding, and positive
outcomes
f) Reviews all OASIS assessment data and documents for accuracy and timeliness; confirms appropriate problem
identification and Plan of Care, development, including disciplines, frequency and duration of planned services
g) Supervises and directs team assistant in accepting referrals, contacting multi-disciplinary field staff, coordinating
patient visits, and maintaining patient records
Maintains current and accurate patient records in accordance with company and governmental guidelines
h) Provides on-call services on a rotating basis, and back-up during emergencies involving clinical staff members
and clients
i) Oversees the quality and timeliness of all clinical documentation to ensure reimbursement for service by Medicare
and other third party payors
j) May perform direct patient care under discipline practice standards
2. Manages clinical staff for designated territory, maintaining effective employee relations in compliance with agency
standards
a) Participates in the selection and evaluation of clinical field personnel
b) Assures clinical skills of field staff, i.e., in-services, joint visits, competency evaluations, multi-disciplinary care
management, and case conferences
c) Assures that staff practices for all services provided meet or exceed all criteria governing the acceptance of
patients, development of health care plans, medical supervision, conformance, and review of physician orders
d) Utilizes all appropriate opportunities during care episode management to maintain and improve personnel
productivity and effective care coordination processes
e) Administers employee relations functions for each discipline, such as disciplinary action and termination
f) Maintains accurate documentation for employee records
g) Monitors personnel compliance to documentation requirements for quality, timeliness, and reimbursement, and
provides feedback VP of Clinical Operations
h) Assures compliance with federal, state, and local statutes regarding competency evaluations and supervisory
visits
3. Coordinates with Branch Administrator to monitor employee practices in providing services
a) Cooperates with all agency staff and management pursuant to mission, philosophy, and objectives
b) Communicates with field staff in the implementation and documentation of agency policies and procedures that
reflect quality and delivery of home health care services
c) Upholds the implementation of standards of patient care and nursing practices, and the evaluation of standards
on regular basis
d) Communicates changes in agency operations and policies/procedures to team personnel in a timely manner
e) Assures consistent and concise lines of authority and responsibilities are interpreted and maintained among
branch staff members
f) Maintains systems to monitor employee compliance with regulations and standards
g) Identifies clinical and non-clinical areas needing improvement; offers suggestions for system redesign; tests
solutions for improvement
4. Completes regular reporting activities, assists Administrator in making optimal operational decisions
a) Maintains administrative records of team operations and client cases
b) Assists in establishing, promoting, and maintaining professional and cooperative business relationships with
governing agencies, local businesses, the medical community, and educational venues
Attends agency meetings and makes formal presentations when requested
5. Participates in personal and professional growth and development
a) Responsible for providing positive model for team through effective self-management and leadership
b) Maintains current knowledge of industry guidelines
c) Maintains own clinical skills and competency to be a resource to staff
d) Ensures all staff meets regulatory and policy requirements for education
e) Updates staff on changes in healthcare procedures
Seeks additional training/education as determined by self-assessment and manager
6. Fulfills additional performance responsibilities as assigned and as necessary
a) Willingly accepts additional job responsibilities, as appropriate, to meet business goals/objectives
Demonstrates flexibility in order to maximize the number of hours worked
b) Performs the related duties as needed, i.e., patient visits, risk management, etc.
Requirements

- Acceptable health status, as defined in agency policy.
- Education and/or Experience: Must be a registered nurse or licensed therapist, currently licensed in the state of agency operation
- Graduate from an accredited college with a bachelor’s or master’s degree in nursing. Additional years of experience may be considered in lieu of education.
- Must have a minimum of five years of experience, with one year of home healthcare experience, and at least one year of supervisory experience, preferred
- Must have one year of specialty experience in specific discipline if assigned to niche service
- Must have knowledge of/experience with managing skilled services delivery and operations
- Knowledge of federal and state regulations, as well as Medicare regulations for home health services
