The Assessment Nurse (AN) assists in planning, implementing, and evaluating nursing care with the Resident Assessment Instrument (RAI) coordinator. If the AN is a Registered Nurse, the AN may be designated as RAI coordinator. The AN assists with the pre-admission screening process, admission process and nursing interventions. The AN is responsible for clinical documentation to include observing, recording, and reporting to the appropriate person the signs and symptoms that may be indicative of a change in the resident. The AN communicates with the physician, nurses, nurse aides, interdisciplinary team members, residents and resident representatives as to resident’s condition or response to treatment. The AN follows and completes the RAI process including documentation requirements, Minimum Data Set (MDS), Care Area Assessment process, care plan development, and care plan implementation. The Assessment Nurse is responsible to the Director of Nursing.
Qualifications
1. At least one year of long term care experience
2. Demonstrates leadership and managerial ability balanced by strong clinical knowledge and skills.
3. Has the ability to organize, plan and supervise the Nursing Department in coordination with other resident services.
4. Maintains effective interpersonal relationships and works well with the general public.
5. Is capable of implementing changes as mandated by Federal, State and management recommendations.
6. Must be willing to be in and/or available to the facility weekends as well as weekdays.
7. Must be willing to participate in the on call schedule.
8. Must demonstrate the ability to function as a cooperative team member with all disciplines
Responsibilities
1. Interacts with discharge planners and staff from referring facilities, agencies and hospitals to pre-screen potential residents for admission and readmission to the facility as directed by supervisor.
2. Assists/performs screenings and/or evaluations of new admissions, pre-admissions, and re- admissions utilizing data collection and recording.
3. Assist with the management and collaborates with the MDS Team on pre-admission screening, assignment of observation and look-back periods for the MDS completion including the implementation of a “significant change” MDS/plan of care processes.
4. Performs data collection for the MDS process on residents and reassessments as per the MDS scheduled requirements unless these are assigned to other clinical disciplines for completion.
5. Assists in development and revision of plan of care as indicated by the resident’s response to treatment and the effectiveness of the overall plan of care in collaboration with the interdisciplinary team.
6. Communicates appropriately and clearly to the RAI Coordinator and other interdisciplinary team members (IDT) regarding the resident’s plan of care.
7. Consults other departments as appropriate to provide for an interdisciplinary approach to the resident’s care.
8. Demonstrates an ability to be flexible, organized, and functions under stressful situations.
9. Treats residents and their families with respect and dignity; ensures resident confidentiality and privacy.
10. Interacts professionally with resident/family and involves resident/family in the formation of the resident-centered plan of care.
11. Promotes effective work relationships with the administrator, medical staff, personnel, volunteers, agencies, and management staff affiliated with the facility
12. Assist with investigations and follow up of incidents, accidents and grievances.
