A health services organization in New York is looking to add a licensed Registered Nurse (RN), Licensed Master Social Worker (LMSW), and/or Licensed Clinical Social Worker (LCSW) to join their growing team in Rockland County as a Quality Coordinator.
Responsibilities:
The Quality Coordinator (RN / LMSW / LCSW) will be responsible for:
Facilitating improvement of inpatient and outpatient medical record documentation for compliance of quality indicators by intervening, interacting with, and educating Physicians, Nurses, and other licensed independent practitioners
Concurrent and retrospective data abstraction for core measures
Performing other duties, as needed
Qualifications:
6 years of Hospital Nursing experience
Bachelor's Degree in Nursing and/or Master's Degree So
NYS licensed Registered Nurse (RN), Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW)
Experience in Chart Reviews and Data, specifically core measures
Knowledge of Joint commission
Experience in Quality and Process Improvement
Experience with Data Aggregation tools
Solid problem solving and time management skills
Great interpersonal skills
Excellent communication skills (written and verbal)
