JOB DESCRIPTION:
The Director of Case Management provides strategic and operational direction and leadership in building a coordinated care delivery system in collaboration with UVMMC, UVMHN, Transitions of Care Team and community partners. The incumbent is accountable for developing and implementing Case Management programs, education, services and performance improvement plans throughout all areas of the Medical Center and ambulatory care sites. The incumbent is the leader of the organizational Length of Stay (LOS) initiatives in collaboration with key leaders across the medical center and the UVMMC Transitions of Care Program.
This position leads the facilities Case Management and Social Services, including but not limited to clinical resource management, discharge planning activities, clinical social work and best practice in medical necessity determination and concurrent review, and capacity management. Directs and manages areas including planning, organizing and directs all activities, staffing, performance improvement in the delivery of clinical services (such as LOS reduction). Reports needs within the Medical Center, as well as government and regulatory reporting. Guides Care Coordination activities according to the needs, requirements, and policies of the Medical Center, Supports population health initiatives and any Federal and State agencies, according to standard practices of the professions under the director's accountability. Works closely with all departments at the medical center and the post-acute service providers to streamline the patient transition through the health care system and into the community post discharge in alignment with the UVMMC Transitions of Care Program.
EDUCATION:
Education: Masters degree in nursing or experience equivalent required.
Licensure: Current Registered Nurse (RN) license in state of Vermont. CM certification preferred.
EXPERIENCE:
Five or more years of progressive leadership experience in the health care industry with a focus on key areas such as care coordination, case management with quality or performance improvement experience.
Experience working in multi-hospital healthcare systems including academic medical centers is preferred.
Demonstrate experience working collaboratively and laterally with a team and as part of a bigger team at the system level.
