What are the responsibilities and job description for the LPN Wound Care Nurse position at The Vein Company?

As a Wound Care Nurse, you will be responsible for providing support to ensure efficient operation of the wound care clinic. You will support doctors and patients through a variety of tasks related to patient care management, organization, and communication. The target is to complete all activities accurately, with high quality and in a timely manner.

Responsibilities and Duties

  • Responsible for assessing skin alteration, implementing orders and treatments.
  • Evaluating progress of specialized skin and wound conditions.
  • Responsible for identifying wound types and analyzing trends.
  • Performs wound care treatments as ordered.
  • Performs all preventive skin care, dressing changes, and wound care.
  • Prior experience applying compression wraps and education on compression garments.
  • Reports and communicates findings and diagnostic tests with the assigned physician.
  • Assist with wound education efforts with staff members.
  • Educate patients and families on wounds and wound care topics.
  • Maintain clean, orderly treatment cart and restock treatment supplies.
  • Review patient schedules, organization of patient flow, accompanying patients to exam/procedure rooms, performing vital signs, preparing exam rooms, performing screenings per provider guidelines, assisting providers with various procedures, charting, relaying instructions to patients, and answering phone calls in an efficient and timely manner.
  • Performs clerical functions as assigned which may include but are not limited to the following:
  • Sending/receiving patient medical records, obtaining radiology reports, obtaining office notes from primary care providers, documenting patient communication in patient charts, completing forms/requisitions ensuring accuracy in completeness, scheduling appointments, ensuring accuracy of patient demographics, and updating charts to ensure that information is complete and accurate.
  • Respecting/promoting patient rights and privacy, responding appropriately to emergency situations, returning patient calls in a timely manner, be in compliance with HIPAA and PHI laws.
  • Performs clinical medical assisting functions that vary according to state law, which may include but are not limited to the following: Checking vital signs, procedure preparation, clinical pharmacology, medical terminology, prescription verifications with physician’s orders, bio hazard waste disposal and monitoring, laboratory procedures according to OSHA guidelines and quality control methods.

  • Demonstrates effective customer service skills and communication skills (verbal and written).
  • Demonstrates effective time management skills including priority setting.
  • Ability to multi-task in a fast-paced setting.
  • Ability to interact with diverse groups of patients, member of the public and other healthcare providers.
  • Ability to follow instructions as directed and to ask for clarity if not understood.
  • Wound Care Certification Preferred 2 to 3 years of experience.
  • Must possess a current CPR certification and maintain certification.
  • Travel required.
