We are hiring for Physician Assistants and Nurse Practitioners to serve in our skilled nursing facilities. This position will work closely with a multidisciplinary team to provide care to nursing facility residents and continuously improve processes and quality of care.

Physical Rounding will be conducted in a post-acute setting.

Job Description

  • General Post-Acute/SNF knowledge or experience preferred
  • The provider is responsible for rounding daily and prescribing medications and treatments
  • The provider will use PointClickCare and secured messaging to answer patient questions from nursing staff during shift time
  • Physician will provide high quality patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;
  • Documenting notes in the EMR systems - PointClickCare and Gehrimed;
  • Maintaining detailed notes of appointments with patients, including comments, tests and/or treatments prescribed, and test results and updating patient charts on an ongoing basis, in no case more than 48 hours after the patient encounter;
  • The provider will be available for phone consultations with facility nursing staff regarding patients at the facilities we cover;
  • Consulting with patients to understand their symptoms and health concerns;
  • Diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes;
  • Prescribing or administering medication, therapy, and other specialized medical care to treat or prevent illness, disease, or injury;
  • Explaining procedures and discussing test results or prescribed treatments with patients;
  • Monitoring patients' conditions and progress, and re-evaluating treatments, as necessary;
  • Providing health and wellness advice to patients, including diet, physical activity, hygiene, and disease prevention;
  • Immunizing patients against preventable diseases;
  • Referring patients to other medical specialists, when necessary;
  • Demonstrating leadership abilities, with an ability to communicate with persons from all educational and cultural backgrounds;
  • Demonstrating tact, initiative, resourcefulness, a professional manner, enthusiasm, and a non-judgmental attitude;
  • Maintaining confidentiality of all patient information according to federal guidelines and regulations;
  • Treating patients respectfully and not discriminating against patients on the basis of race, color, national origin, religion, sex, disability, or sexual orientation.

Job Requirements:

  • Candidates must hold a valid license in the State of Michigan without any disciplinary action.
  • Candidates must hold a valid degree from an accredited institution.
  • Candidate must be familiar with EMR and charting practices
  • Candidate MUST NOT have any criminal history whatsoever.
  • Candidate must have DEA registration

COVID-19 Vaccination Requirement: Please note that this position requires in-person work at skilled nursing, rehabilitation, long-term care or post-acute facilities that participate in Medicare and Medicaid programs. In recognition of the potential health threat that the COVID-19 virus poses to the population Theoria Medical serves and in compliance with the Omnibus COVID-19 Health Care Staff Vaccination Emergency Rule, Theoria Medical requires that all applicants provide proof that they are fully vaccinated prior to providing care in any facility Theoria Medical services. If you are not currently vaccinated but are interested in the position, you may still apply but will need to provide proof of having received the first dose of a two-dose COVID-19 vaccine on or before December 5, 2021 or proof of a one-dose (Johnson & Johnson) vaccine. You will also be required to provide proof that you have received the second dose of a 2-dose COVID-19 vaccine on or before January 4, 2022.

Acceptable forms of proof include: CDC COVID-19 vaccination record card (or legible photo of the card); documentation from a health care provider or electronic health record; or State immunization information system record. Those vaccinated outside of the U.S. may present a reasonable equivalent of the previous examples.
