Position Summary: Under the direction of the rehabilitation director and therapy staff, assists the therapists and therapist assistants in a non-clinical capacity in regards to the productive operation of a therapy department. The Rehabilitation Transporter/Aide must demonstrate competency in skills and techniques, as taught by the Rehabilitation Department necessary to assist therapy staff in their day to day responsibilities.
Qualifications: The Rehabilitation Transporter/Aide must hold a high school diploma or the equivalent. Strong organizational skills and clerical and clinical experience is preferred.

· Assist unit staff with transfer of residents/patients as necessary.
· Transports residents and patients to/from therapy treatment areas.
· Monitors residents and patients during exercise routines.
· Assists with set-up/clean-up of treatment areas.
· Assists with the cleaning and distribution of wheelchairs.
· Maintain equipment and make minor repairs.
· Assists with clerical duties (i.e. copying, faxing, filing, answering phones, etc.).
· Keeps work area neat and orderly.
· Maintains residents' and patients' safety during transport and while assisting in treatment area.
· Responds well to instructions and guidance, finds solutions to work related problems, and completes assignments independently, not requiring constant supervision.
· Attends and participates in required and MANDATORY in-service educational programs, on the job training programs, etc. as directed and/or scheduled.
· Answer phone, fax, email/distribute rehab information to other disciplines as directed.
· Coping and filing of therapy documents.
· Monitoring therapy supply levels and equipment utilization.
· Fax/Email equipment orders for patients returning home. May be done via Parachute or other electronic method.
· Monitoring and coordinating equipment maintenance.
· Performs other related duties as assigned.

· Maintains an adequate liaison with families, residents and patients.
· Demonstrates awareness, respect and concern for internal customers (coworkers) and external customers (i.e. patients, residents, families, physicians, referral sources)
Residents' Rights:
· Maintains CONFIDENTIALITY of all resident care information to assure resident rights are protected.
· Assures that the resident's rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are followed.
Attendance:
· Observes rules, policies and regulations with regard to attendance, punctuality, appearance and professional attitude as established by the facility.
· Due to the nature of the industry, many of our programs offer seven (7) day a week services. Accordingly, you may be asked to work weekends as part of your scheduled workweek
Miscellaneous:
· Conforms to the HIPAA Compliance Program and applicable facility policies for patient privacy.
· Conforms to the facility’s policies and procedures.
Physical:
· Must have the ability to follow instructions as to the plan of care for each resident assigned.
· Must have good mental and physical health, sound judgment and high moral standards.
· Must be able to climb stairs, bend, maneuver equipment, push wheelchairs and small carts.
· Must be able to verbally communicate with staff and residents; visually assess residents’ needs and problems; read and write.
· Must have sincere desire to work with the aged and those with limited capacity for self care.
· Must have the ability to deal with residents with compassion and patience.
· Must demonstrate decision-making skills relative to resident care and safety.
· Must be able to follow instructions from the Rehabilitation Department
· Must be able to document clearly and concisely on required forms.
Emotional:
· Must have the willingness to work with severely physically, emotionally and cognitively compromised residents and patients.
· Must be able to take direction from registered therapist and other team members to promote patient treatment and safety.
· Must be willing to work in an environment with risk of personal injury, exposure to infectious waste/materials, infectious diseases, and odors.
· Must be able to cope with the emotional stress associated with working with patients, families and coworkers.
· Must be able to work independently with periodic (daily or less) support and direction.

· Must be able to shift attention and task performance based on patient and facility requirements.
