The Aging Services Specialist is responsible for performance of case management activities for non-Medicaid clients over 60 years of age, consisting of screening, assessment, care planning, determining eligibility for program subsidies, arranging and authorizing service delivery, services follow-up, and client monitoring, reassessment, and discharge.

Pay rate: $17.50/hour
Work Hours: Monday - Friday, 35 hours per week.
Location: Owego, NY

Job Requirements:
  • Provides program services, outreach activities, short-term casework, and other direct services of an informational, educational, and supportive nature to meet various needs of older persons and their families
  • Prescreens applications for assistance and verifies information is complete
  • Works closely with other staff and organizations to coordinate the delivery of services
  • Interviews clients to assess their service needs, assists in locating and/or obtaining services, assists in completing applications for assistance
  • Interviews older persons, or their designee, to assess their economic, emotional, social, and environmental needs and evaluate them for services
  • Provides information, referral, and/or assistance for services and benefits to meet the needs of older adults and their families
  • Completes reassessments as directed
  • Maintains accurate and current records regarding client contacts and activities including on-going narrative files for each client, outcomes, and follow-up activities, and daily and monthly numerical reports
  • Develops knowledge of other community resources in order to make appropriate referrals and maintain working relationships with various community agencies to which individuals are referred for services
  • Screens and conducts face-to-face in-home assessments and reassessments to identify an individual’s problems and care needs in the major functional areas, including information necessary to determine the individual’s functional level and to identify unmet care needs
  • Verifies eligibility for or assists individuals in applying for other programs or services such as Title XVIII, XIX, or XX of the Federal Social Security Act and to determine a need for and, if necessary, arranges for the receipt of EISEP ancillary services
  • Develops service plans in conjunction with individuals or their authorized representatives to address individual’s problems and care needs – arranges for additional medical, nutritional, mental health, or housing assessments to be conducted as needed from appropriate collaborative agencies
  • Authorizes and arranges for EISEP services with ongoing monitoring of service provision based on client needs
  • Discharges clients from the program with assistance in seeking appropriate follow-up care if necessary
  • Maintains case records in accordance with EISEP regulations and prepares reports and documentation as requested by office
  • Periodically reviews cases to determine changes in the individual’s or family’s situations affecting need for service
  • Prepares a variety of reports for case records, courts, referrals
  • Assesses needs and arranges support services that may be needed for clients being served
  • Facilitates client’s referral to needed services
  • Conducts assessments and/or reassessments, develops care plans, authorizes services, or terminates/discharges clients from the program
  • Identifies the older person’s problems and care needs in the major functional areas, including information necessary to determine the individual’s functional level and to identify unmet care needs
  • Reviews case records for available information for use in formulating a care plan
  • May provide casework counseling in a number of areas
Minimum Education and Experience:
  • A graduate from a regionally accredited or New York State registered four year college or university with a Bachelor’s degree AND one year of full-time paid experience in human services or a related field; OR
  • A graduate from a regionally accredited or New York State registered two year college with an Associate’s degree AND three years of experience in human services or a related field; OR
  • Graduation from high school or possession of a High School Equivalency Diploma AND five years of experience in human services or a related field; OR
  • An equivalent combination of training and experience as indicated above.
Knowledge, Skills and Abilities:
  • Understanding and empathy for the characteristics, needs, and problems of older adults
  • Ability to work with other community agencies, facilities, and services that may be utilized to help older adults
  • Must have computer skills to perform efficiently, e.g. MS Office
  • Must possess strong verbal, written, and interpersonal communication skills including the ability to respond to client inquiries and concerns through effective communication; patience to communicate with people who may have physical or language difficulties
  • Ability to interpret company policies and procedures, and implement accordingly
  • Ability to maintain successful relationships with people both within and outside the agency and gain their cooperation
  • Ability to counsel individuals in the areas of economic, social, and emotional problems
  • Ability to observe, analyze, prepare, and maintain accurate records and reports

  • On-site attendance is a requirement of this position, and the employee must attend mandatory trainings, some of which may be out of town
