What are the responsibilities and job description for the RN Supervisor (PRN) position at Together at Home Personal Care Agency LLC?
We have an immediate PRN opening for an RN Supervisor for Washington and Ozaukee counties. This position works in client homes and is paid per visit for each admission and/or supervision of a client.
The following list of duties is a guideline of possible duties to be performed in the course of working with the client. The RN’s duties are not limited to those listed.
- Evaluate the need for services and make referrals to other services as appropriate.
- Utilize professional assessment skills to identify physical, mental, and psychosocial needs of clients.
- Develop a plan of care for the client, giving full consideration to the client’s preferences for service arrangements and choice of personal care workers, interpret the plan to the PCW.
- Supervise each client and review the care plan at least every 50-60 days
This position will be responsible for approximately 3-5 clients in the these areas.
Qualifications:
- Current licensure as a Registered Nurse under s. 441.06, Wis. Stats.
- At least one year of experience in home health or in home personal care services or willing to train for one year under RN Supervisor
- Valid driver’s license and reliable transportation
- Ability to work independently.
