The successful candidate will be responsible for managing the Minimum Data Set (MDS) information and overseeing the accuracy and timeliness of assessments and care plans. Responsibilities
- Conduct and coordinate the development and completion of the resident assessment (MDS)
- Maintain accurate and timely records of resident assessments
- Ensure compliance with all state and federal regulations related to MDS assessments
- Develop and implement care plans that are individualized to meet the needs and preferences of each resident
- Collaborate with other healthcare professionals to ensure that resident care is coordinated and comprehensive
- Assist in staff education and training related to MDS and care planning
- Participate in interdisciplinary team meetings to discuss resident care and progress
- Current RN license in the state of [location]
- Minimum of 3 years of experience in long-term care nursing, with at least 1 year of experience in MDS coordination
- Proven ability to manage multiple tasks simultaneously while maintaining attention to detail
- Strong leadership skills and the ability to work effectively within a team environment
- Knowledge of state and federal regulations related to long-term care facilities and MDS
- Excellent communication and interpersonal skills
