Position Purpose: Care Engagement is responsible for supporting the organization’s goals of obtaining health needs screenings, scheduling preventive service appointments, and educating members on plan benefits and services. Provide members with educational materials and carry out strategies to increase healthcare adherence and reduce barriers to care.
- Member outreach to discuss medication adherence.
Responsibilities:
- Minimum of 50 outreach calls/day.
- Successfully assist members filling the prescriptions.
Required Skills & Education:
- 1 years experience in retail pharmacy.
- 1 years Call center or other high pressure sales environment, or experience in member, provider, customer service, or call center.
- 1 years of experience with Medication Adherence (conducting medication adherence assessments, identifying barriers to medication nonadherence, and coordinating prescription refills and renewals with prescribers and pharmacies) and basic knowledge of medication quality measures for Medicare and Medicaid.
- Bilingual-Spanish.
- HS Diploma.
