Touchstone Health Services is looking for compassionate, dedicated candidate in Maricopa County area who want to empower youth and families by providing outpatient therapy in the home, clinic, school, and via telehealth services.
Duties and Responsibilities (Including, but not limited to)
Provide direct clinical treatment using evidenced based modalities and best practices.
Receive regular training, professional development, supervision.
Facilitates and participates in Child and Family Team (CFT) meetings according to the AZ Practice Model
Conducts annuals, assessments, reports, and case management as needed.
Completes discharges and transfers in a timely and efficient manner.
Completes all required clinical documentation and reports within 24 hours of the service date; updates service member documentation as appropriate to maintain an accurate Electronic Medical Record; maintains record compliance in accordance with regulatory and agency expectations including (but not limited to) assessments, demographics, consents, and treatment plans.
Responds to crisis in a timely manner according to determined procedure.
Acts as liaison between Touchstone and collaborative agencies.
Bills according to productivity standards
Performs other duties as assigned.
