Position Summary:

 

The position will be independently obtaining medication and allergy histories for patients that will be admitted to the hospital from the Emergency Department.  Information will be obtained by interviewing the patient, patient family, calling local pharmacies and also using other available resources.  The information obtained will be entered into a computer system and it will be used by physicians and other health care providers to determine treatment plans and medication orders during and post discharge from the hospital.

 

Major Position Responsibilities:

  • Primarily responsible for obtaining an accurate and reliable medication history upon admission enabling the continuity of appropriate drug therapy and ensures the patients care is seamless when transferring from home to hospital or from one healthcare facility to another.
  • Information is obtained from a variety of sources (i.e. outside pharmacy, medication administration records, etc.) then verified through patient and/or family interviews.  Enters patient allergies and medication history accurately in electronic medical record.
  • Identifying discrepancies in the patient’s medical record including, conflicting allergy information, changes or duplication of medications entered on previous admissions and working with pharmacists and nurses to resolve discrepancies.
  • Communicate clearly and effectively both in person and on the phone to a variety of audiences including patients, patient care providers, physicians, pharmacists and staff.
  • Assists staff pharmacists and director of pharmacy in all functions to maintain smooth operation of the department.

Safety and Infection Control Responsibilities:

  • Responsible to maintain a safe and clean work environment, including unit based specific safety and infection control requirements.

Qualifications: ESSENTIAL COMPETENCIES, KNOWLEDGE, & EXPERIENCE

  • One to three (1-3) years retail pharmacy experience, preferred.
  • At least six (6) months hospital pharmacy experience, preferred.
  • Cerner experience and computer skills, preferred.
  • Brand/generic name knowledge, required.
  • Excellent communication and interpersonal skills, required.

Education:

  • High School Diploma or GED required.
  • Two (2) years of college or equivalent training in a health related field preferred.

Licenses:

  • Current California Pharmacy Technician, required.

Certification:

  • NVCI within 90 days of hire or transfer, required.

Each new hire candidate who is offered employment must pass a physical evaluation, urine drug screen and pre-employment background checks before starting work. 

 

*Salary/Hourly wage range for this position is posted.  Actual pay will be determined based on verified experience as well as internal equity.
