Aging Waiver Support Coordinator Opportunity
We are seeking a compassionate and experienced Aging Waiver Support Coordinator to join our team at Trimed HealthCare. As a Aging Waiver Support Coordinator, you will play a vital role in supporting individuals with disabilities and seniors in their homes and communities.
Key Responsibilities:
- Provide case management and supports coordination services to individuals with disabilities and seniors.
- Develop and implement individualized care plans to meet the unique needs of each client.
- Collaborate with healthcare providers, family members, and other stakeholders to ensure seamless care coordination.
- Monitor and evaluate client progress, making adjustments to care plans as needed.
- Ensure compliance with state and federal regulations governing Home and Community Based Waiver Services.
Requirements:
- At least 1 year of experience in case management and supports coordination in Home and Community Based Waiver Services.
- Bachelor's Degree in Social Sciences or related field.
- Experience with SAMS and HCSIS systems.
