JOB SUMMARY:
Under the supervision of the Center Director , assesses participant nutrition al status of PACE participants. Provides nutrition counseling, education and therapy to participants , families and employees of Mercy LIFE . Coordinates with contracted meal services to ensure meals meet the needs of the PACE participants and kitchen facilities to ensure standard s are met .
PRIMARY JOB RESPONSIBILITIES AND DUTIES:
Conducts initial and annual comprehensive nutritional assessment on all Mercy LIFE participants . Coordinates with the interdisciplinary team to develop a comprehensive care plan for each participant .
Functions as a member of the interdisciplinary team. Maintain s regular attendance at and participates in interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for 24 hour care delivery. Actively participates in the Intake and Assessment meetings.
Monitors monthly weights, medical diagnosis, drug nutrient interactions, changes in chewing, swallowing, mood changes, hydration status, intake, skin breakdown, reports changes to the interdisciplinary team and makes recommendations for changes to appropriate staff.
P erforms initial and annual competencies on the Dietary Aide(s ).
Oversees maintenance/cleanliness and orderliness of serving kitchen. Monitors portion control; appropriate serving of food; adherence to therapeutic diets, purchasing and inventory of food.
Acts as liaison with contracted meal service to ensure meals meet the needs of the PACE participants.
Provides nutritional counseling and education for PACE program participants and/or caregiver s using appropriate materials. Provides individual and group education as appropriate.
Provides education and training to employees of Mercy LIFE as needed.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES :

Knowledge normally acquired by the completion of Baccalaureate degree from an accredited College with major studies in food and nutrition or dietetics.
Must be a Registered Dietitian in the Commonwealth of Massachusetts .
A minimum of one year’s experience in working with the frail or elderly population. Shall have either training or related experience in the job assigned. The Dietitian shall be knowledgeable of the physical, mental, and social needs of the elderly .
Must have a valid driver’s license, proof of insurance and have means of transportation.
Skills and k nowledge required may include :
Working knowledge of the PACE and Massachusetts ambulatory care regulations.
Working knowledge of MA healthcare and aging networks.
Demonstrated effective leadership and decision-making skills.
Effective supervisory skills.
Ability to foster collaborative working relationships.
Working knowledge of quality improvement and care management systems.
Ability to apply creative problem-solving skills to complex issues.
Ability to effectively and efficiently plan, prioritize and follow-up on delegate responsibilities.
Grasp of the knowledge base from which various disciplines assess the needs of the elderly population.
Effective written and oral communication skills.
Working knowledge of local health care and geriatric service networks.
Working knowledge of spreadsheet, database, word processing, and EHR.
WORKING CONDITIONS :
Exposure to injury, occupational or contagious disease hazards which requires routine precautions.
May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.
Rapid judgment demands and flexibility in scheduled hours including weekend and evening responsibilities.
Some stres s due to work with colleagues, participants and families.
Some local travel may be required.
REPORTING RELATIONSHIPS:
Reports to the Center Director.
Has no responsibility for the direct supervision of any other employees. Will assist in leading the work of Dietary Aide(s) on a daily basis .
