St Mary's Hospital a member of Trinity Health Of New England is looking for an Extended Care Facility (ECF) Transfer Coordinator to join our Case Management team. Our vision is to provide state-of-the-art healthcare to those within our local communities, creating a positive impact for each individual that walks through our doors.The ECF transfer Coordinator organizes of all aspects of a patient’s referral and transfer to an extended care facility, including the maintenance of tracking and documentation and the collaboration with other members of the multidisciplinary team.

Work Hours/Shift:
- Full-Time, Monday – Friday
You Will Be Responsible For:
- Exceptional Patient Care: Maintains open communication with the patient, family, Nurse Care Manager, medical staff, insurance company and all others directly involved with the referral of the patient.
- Excellent Communicator: Collaborates and communicates with staff, patients and families to facilitate effective transfers to appropriate facilities. provides answers to all clinical questions on referrals made by ECF admission coordinator and ECF nursing staff.
- Technical Familiarity: Maintains proficiency with office computer systems. Performs administrative duties in a fast-paced environment
- Active Teamwork: Promotes collaborative relationships with ECF’s and community agencies through open, honest and professional communication and problem solving.
Requirements:
- High School/GED Required, Associates Degree Preferred
- Previous Healthcare experience Preferred
- Knowledge of medical terminology Preferred
- Strong organizational, verbal, and writing skills required.
- Experience with Excel, PowerPoint, Word required
