Description:
Provides professional nursing care to the organization’s clients as prescribed by the physician, and/or requested by the client/family, and in compliance with the state’s Nurse Practice Act, any applicable licensure/certification requirements, and the organization’s policies and procedures. The RN is accountable for the client assessment, care planning, therapeutic intervention, and achievement of client outcomes. Provides functional support to Home Healthcare Aides and LPNs as needed.

ESSENTIAL FUNCTIONS
1. Effectively and efficiently prioritizes visit content and plans visits based on client capabilities, condition, preferences (when possible), physician orders and available resources.
2. Maintains a current working knowledge of applicable Federal. State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical and professional behavior.
3. Accurately assesses clients at admission, at required time points, and as needed, utilizing appropriate standardized documentation. Assesses the client ‘s condition and needs and determines the agency’s ability to meet those needs in the home care/hospice setting.
4. Informs the client/caregiver about the agency, care/services available, client obligation for payment, and other responsibilities and rights, before care is initiated.
5. With the physician, office care manager and client, develops appropriate assessment-based plan of treatment including: attainable episode goals and agency services needed to improve and/or stabilize client’s medical condition and promote independence. Participates in timely revision of the plan of treatment as appropriate.
6. Makes appropriate referrals for evaluation/care to other disciplines and services, and assists in coordinating care with others to ensure effective and efficient care is provided.
7. Utilizes interview, observation and evaluation in assessing clients and applies nursing judgment, consistent with practice standards, in formulating nursing interventions and making recommendations to the physician and client.
8. Reports changes in client condition as appropriate and in a timely manner, to the client’s physician and/or Case Manager/designee, and obtains orders for changes in the plan of treatment to respond to the client’s condition.
9. Initiates and assures implementation of procedures, orders for medication and treatments as prescribed by the physician in accordance with the plan of treatment.
10. Accurately documents observations, interventions and evaluations pertaining to client care management and services in accordance with agency policy and procedure utilizing the point of care information system. This will include: assessments, visit reports, medication information, treatments, coordination of services, client progress or lack of progress toward goals, and contacts with physicians and agency and community providers.
11. Takes appropriate steps to assure timely submission of client data to the office for inclusion in the client record (by timely electronic transmission of data, or when appropriate by fax or mail).
12. Works with scheduling to assure visits and client care coverage are scheduled in accordance with the plan of care, maintains an appropriate productivity level and are suitably assigned to other team members when needed. (Weekly schedule, sup visits, weekend & on-call)
13. Initiates and maintains communication regarding the client’s care/needs with the physician, clinical supervisor, other disciplines, and family, as appropriate.
14. Updates other disciplines regarding changes in the plan of care and the client’s condition when indicated; and coordinates referrals and recertification information with other disciplines.
15. Notifies physician of changes in the client’s condition and/or need for additional orders, obtaining and documenting verbal orders per policy.
16. Participates in case conferences when indicated with other members of the home care team.
17. Demonstrates furthering professional growth through sharing knowledge with others (e.g. presenting at agency and/or community in-services, acting as preceptor to others).
18. When acting as primary nurse on the case, takes lead responsibility for care plan and case management, assuring appropriate care, supplies, and follow through.
19. Develops/updates the HHA plan of care, when needed, and assures the HHA is oriented to the client and care plan.
20. Makes bi-weekly supervisory visits to the client to assess quality of aide services provided and need for HHA care plan revisions, offering education to the HHA as needed and feedback as appropriate for staff evaluations.
21. Supervises LPN care and competence monthly or more frequently as needed to accomplish client goals.
22. Participates in the orientation of new employees, preceptorship of new RNs and the educational process of nursing students as appropriate.
23. Ensures that services are provided in a manner that supports continuous quality improvement, customer-oriented focus and quality client care outcomes.
24. Ensures that services are provided in a manner that supports a quality work environment for employees, as measured by participation in an employee satisfaction survey process.

QUALIFICATIONS
1. Active registration for Registered Nurse - Board of Nursing, State of Iowa or Illinois
2. An Illinois or Iowa (whichever is not currently held) Registered Nurse – Board of Nursing, license must be applied for within 30 days of hire and secured within 6 months of hire.
3. Associate’s degree or diploma from an accredited school of nursing.
4. Baccalaureate degree in nursing preferred; must obtain within 6 years of hire, effective 04/01/2012.
5. Minimum of one (1) year experience as a professional acute care nurse preferred.
6. Home healthcare experience preferred.
7. Valid driver’s license, current auto insurance and reliable transportation to and from work site are required.
8. Must be comfortable operating in a collaborative, shared leadership environment.
9. Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission and vision, goals, and values of Trinity Health.
PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS
1. Frequently lift, stands, and walks (35% of time)
2. Occasionally bend, stoop, and twist (10% of time).
3. Occasionally position and lift patients (10% of time).
4. Ability to perform continuously detailed tasks with frequent interruptions and work reprioritizations (90% of time).

6. Must be able to travel to various TH/THHS worksites or various work locations as required
