RN (Home Care/Long-Term Care) – Eddy SeniorCare PACE – Latham, NY

Eddy SeniorCare is an exciting, innovative model of care for seniors, located at 385 Watervliet-Shaker Road, Latham.  Our Program of All-Inclusive Care for the Elderly is located within Provincial House.  We provide a team-based model of care including a physician, RNs, LPNs, Home Health Aides, physical/occupation/speech therapists, social workers, transportation, a day center team, and more!  Currently most of our patients reside at the Sisters of St. Joseph; we also care for patients in the surrounding community.  This is a truly unique program our region is fortunate to have access to, with only 130 PACE programs across the country!

What you will do:

• Maintain individuals in the community and on site as independently as possible for as long as possible, and strive to reduce avoidable emergency room visits and hospitalizations and improve participant outcomes and quality of life.

Responsibilities:

• Providing oversight and coordination of LPN and Aide services on the night shift
• Providing nursing care as needed to the sisters  
• Collaborate closely with the interdisciplinary care team

What you will need:

Required:

• A current license to practice as a Registered Nurse (RN) in the State of New York
• 2 years or more of recent experience as a RN ideally in long term care
• Valid driver license and reliable vehicle

Preferred:

• 2 years or more of experience in a home care setting
• Bachelor's degree of nursing
• Registered Nurses at Eddy SeniorCare must possess a passion for caring for seniors and demonstrate a respectful, caring, professional demeanor
• The RN must have excellent time management and organizational skills
• The RN must be able to communicate effectively, verbally and written and possess strong critical thinking skills to problem solve
