Pharmacist - Infusion Per Diem

If you are a Licensed (or eligible) Massachusetts Pharmacist that enjoys clinical program development, patient and care team education, coordination care for patients receiving parenteral and/or enteral therapy in the home, and overseeing home infusion pharmacy operations in collaboration with the pharmacy manager of record, this is an exceptional opportunity to make a positive impact on the lives of patients while developing within a quickly expanding sector of health care. Per Diem as needed Monday – Friday between hours of 9 AM – 5:30 PM Responsibilities Ensure Optimal Drug Therapy Participate in the care of patients admitted to the home infusion therapy service (could range between primary care, medical, surgical, and adult or pediatric specialty practice) Participate in the development of departmental drug-specific and population-specific initiatives Detect and report medication errors and adverse drug reactions in a timely manner Provide clinical consultation to physicians, nurses and pharmacists Provide accurate, adequate and timely drug information to clinician and non-clinician home infusion therapy team members Evaluate and document patients’ medication taking behaviors and design strategies for improving drug therapy adherence Coordinate care to ensure safe and effective transitions from inpatient to outpatient care Document pharmacist interventions according to established policies Guideline Development and Implementation Participate in the development and implementation of treatment guidelines, protocols and clinical pathways Policy and Procedure Development and Maintenance Coordinate review of policies and procedures related to clinical practice Identify the need for, and create new policies and procedures Leadership Actively participate in a health system multidisciplinary committee Present at state and national professional meetings Publish in a refereed journal Education Training and Development Provide continuing education for professional staffs at Tufts Medicine member entities (e.g., pharmacists, physicians, nurses) Drug Cost Management Identify changes in drug usage (volume and patterns) based on published literature and Tufts Medicine Pharmacy practices Department of Pharmacy Operational Support Collaborate with other pharmacists to provide service to every patient covered by a pharmacist team. This includes weekend, holiday, evening, night and on-call coverage as described in departmental policies and procedures Experience Must be a graduate of an accredited college or university with a BS in Pharmacy of a PharmD degree Unrestricted license to practice pharmacy in the Commonwealth of Massachusetts or eligibility Required: Comfort with and adaptability to contemporary healthcare workplace technologies including but not limited to electronic medical record(s), software applications, messaging systems, video chat, and team collaboration platforms (e.g. Microsoft Teams, etc.) Preferred: Home Infusion Therapy experience PGY1 Pharmacy Residency Health system pharmacy practice experience Experience with Epic™ electronic medical record Experience with a Home Infusion Therapy software platform Intermediate to advanced level skill in Office Suite (Word, Excel, PowerPoint) Tufts Medicine is a leading integrated health system bringing together the best of academic and community health care to deliver exceptional, connected and accessible care experiences to consumers across Massachusetts.
