Overview
HIRING : Complex Care Coordinator
LOCATION : Newburyport, MA
SALARY : $27-$30 / hr.
POSITION SUMMARY :
The Complex Care Coordinator ensures the continuity and consistency of care for Individuals with intellectual and developmental disabilities residing in community residences.
The Complex Care Coordinator supports the nursing team and other program management by liaising with connected community providers, DDS, and other agency representatives.
RESPONSIBILITIES :
- Ensures timely medical, dental, and psychiatric care is provided to Individuals in their assigned caseload; coordinates medical evaluations and other consultations, as required.
- Maintains Individual’s records, including but not limited to : health care provider orders, protocols, and care coordination documentation.
- Provides ongoing training and in-service education to management and staff as directed by DON.
- Works in a collaborative, team-oriented fashion with the program management and the nursing team.
- Ensures all recommendations and follow-up treatment are completed in a timely manner.
- Knowledgeable about regulations and statutes governing licensure and certifications, MAP; human rights; PBS and any additional regulating bodies.
- Ensures adherence to all DDS and MAP regulations through quality assurance.
- Assist with Policy creation / revision .
- Other duties as assigned.
QUALIFICATIONS :
- Bachelor’s Degree in social work, human services, or related field
- Maintain CPR and First Aid certification
- At least (2) two years of professional experience required; home care and / or community-based residential programming preferred
- MAP certification preferred; prior MAP experience required
- Strong organizational and multi-tasking skills
- Must be able to handle a wide variety of people and situations calmly, professionally, and confidentially
- Demonstrates a high energy level, professional demeanor, and the ability to work independently and as part of a team
- Proficient in Microsoft Office Suite and Google Suite
- Valid driver’s license and willingness to drive to Program locations throughout the North Shore and Merrimac valley
