Summary: Responsible for oversight of all HPC programing including each residential program. Oversight of programming involves assisting with all required Medicaid documentation, participant programming, activities, and support plans for participants. This position is responsible for supervision of staff, ensuring adequate staffing levels in the homes at all times. The position provides leadership and expertise in customer service, behavior support, support of staff and families involved in day to day interactions. In addition, must provide quality care to program participants by implementing the plan of care for the individual.
Essential Duties and Responsibilities include: Operation of computer, phone and office equipment for business/service, light lifting associated with day to day operations, good communication skills and ability to provide transportation.
· Ensures new hires receive site training on all aspects of HPC, reviews the ISPs for each participant and submits the site training form to HR upon completion.
· Responsible for all Medicaid documentation and regular audits of all HPC documentation with regular reports to Management.
· Assists with setting-up new HPC programming which may involve budgeting, shopping, and assistance with relocation of participants.
· Responsible for growing the HPC programming by working directly with SSAs, guardians, and families in the community.
· Responsible for maintaining the current HPC programming to ensure that the programs are successful by monitoring the participants, guardians, and families and open communication with SSAs.
· Provides quality care to program participants in an environment that promotes their rights, freedom of choice and individuality.
· Provides training, support, and supervision to HPC staff.
· Schedules staffing in the home and arranges for staff call-outs, scheduled PTO, etc.
· Must become knowledgeable about the Individual Service Plan (ISP) and provide care according to the participants plan.
· Responsible for maintaining the comfort and privacy of the participants in the delivery of care, which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, transferring, ambulation, and assistance with communicating their needs.
· Performing household responsibilities of the program, which may include assistance with grocery lists in conjunction with dietary needs, grocery shopping, meal planning, cooking, and cleaning of the home.
· Completes necessary documentation according to the standards required by TWi.
· Audits any and all HPC paperwork to verify accuracy prior to submittal.
· Facilitates processing of Unusual and Major Unusual Incidents.
· Must be able to communicate effectively with participants, colleagues, family members, guardians, case workers, and community members.
· Facilitates working relationships with family members and guardians. Addresses employee concerns and makes recommendations for staffing needs to Chief Operational Officer.
· Brings staffing concerns as deemed necessary to COO or HR if unable to resolve satisfactorily.
· Provides crisis assistance for individuals as may be necessary. Assists in training of individuals.
· Responsible for providing transportation for participants to appointments, errands, and community events. This includes walks around the neighborhood for community interaction.
· May be responsible for development of the ISP in coordination with the SSA.
· Communicates with SSA staff for the purpose of coordinating county board programming. Ensures services are being provided in accordance with Individual Service Plan. Works in conjunction with Service and Support Administration staff to ensure communication with individuals, families, and providers.
· Ensures compliance and quality assurance, when necessary, with all applicable federal, state and local rules, regulations, board policies, and accreditation standards.
· Must review Individual Service Plans and accurately monitor hours of service provided. This may include data entry; type of service provided during the course of the day/night.
· Ability to independently problem solves and offer solutions.
· Ability to remain patient and adapt to ever changing situations that may occur.
· Must comply with all policies and procedures set forth by TWi.
· Must be able to work with a team, but also able to work independently.
· Responsible for being a role model by displaying appropriate decorum, dress, hygiene and attendance.
· Fully understands the rights of participants and promptly reports any evidence of abuse or any suspected violation of a participant rights.
· May be required to provide direct care assistance during staff call-offs if there is no other coverage available..
· All other duties as assigned.
Qualifications: HS Diploma or GED. Minimum of 1-2 years experience working with adults with disabilities required or equivalent supervisory experience. Must be willing to obtain the following certifications: Crisis Intervention, First Aid/CPR, and Medication Aid within 60 days of hire.
