Summary
The individual in this position assists the Case Manager to complete referrals for post acute services for patients and assist with tasks that do not require a clinical license or degree. The individual’s responsibilities include the following activities: a) arrange post-discharge services b) create and follow up on electronic referrals using the Tenet Case Management system, c) review patient choice letters with patients/families for required signatures, d) provide follow up Important Message to Medicare patients prior to discharge, e) communicate with patients, families and other members of the care team, f) complete tasks assigned by Case Manager, g) make copies, send faxes and complete phone calls, h) complete process reviews or audits as requested, and i) other duties as assigned.
Qualifications
EDUCATION / TRAINING / EXPERIENCE:
Bachelor’s degree preferred. Acute hospital experience preferred.
LICENSES / CERTIFICATIONS:
None required.
SKILLS / KNOWLEDGE / ABILITIES:
Skills required include excellent organizational skills, excellent verbal and written communication skills, demonstrated problem solving skills, and computer literacy. Basic knowledge of Medicare, Medicaid and other regulatory guidelines preferred. Organizational skills, appropriate for a fast pace environment, able to multitask and handle several projects and maintain deadlines. Excellent verbal and written communication skills also required. Proficiency with computers, in particular the Microsoft Office Suite is required.
Pay range: $18.17- $27.25
Individual wages are determined based upon a number of factors including, but not limited to, an individual’s qualifications and experience
