With direction and guidance from the Director of Care Management, the Care Coordinator provides high quality care coordination to Venice Family Clinic patients.
The Care Coordinator will : * Reach out to patients who have rising risk, high risk and / or social needs and provide care coordination services.
- Conduct a comprehensive high risk assessment with each patient including demographic information and biopsychosocial history.
- Act as patient advocate, and provide intervention and support to patient when appropriate.* Connect patients to resources for basic needs, and provide appropriate information regarding referrals (ensuring linkage).
- Assist patients with scheduling appointments and coordinating transportation to appointments.* Provide disease specific and preventive care patient education.
- Provide administrative support and complete other duties as assigned.Salary Range : $29.64 - $39.13 Hourly
Qualifications
Required : * Minimum of 2 years of experience in a health care or social services setting working directly with patients / clients.
- Knowledge of community resources, health education methods, advocacy, harm reduction and general clinical knowledge.* Bilingual in English and Spanish (oral and written language skills).
- Ability to complete required program documentation, including the use of electronic health records and other software.* Ability to create and maintain boundaries with clients.
- Ability to follow through on assignments, prioritize, and work effectively with people of diverse cultural and educational backgrounds.
