Care Coordinator, UHS School Clinic Franklin United Health Services is seeking a Care Coordinator to join our Care Coordination department at UHS School Clinic Franklin in Binghamton, NY. This position is benefits eligible. Shift Day Hours per week: 40 Salary range: $21.80 - $32.60 per hour, depending on experience. Overview Provides Care Coordination/Care Management services in Mental Health. This position is a key member of the care team for all patients on this position’s case load. Qualifications Education/Experience Minimum Required: Associate’s Degree in a Human Service-related field May accept 5 years of related care coordination experience in lieu of an Associate's Degree..
