Discharge Coordinator

Part-Time

Responsibilities include but are not limited to:  Case Management services including communicating with outside providers and working with parents, outpatient providers, and therapists to establish safe and supportive aftercare plans.

Qualifications

Education/Experience:  Experience in Case Management preferred.  Experience in and/or knowledge of mental health systems preferred.

Education:  Bachelor's Degreen in a related healthcare field highly preferred.
