What are the responsibilities and job description for the DISCHARGE PLANNER position at UHS?
Laurel Ridge Treatment Center is hiring a Discharge Planner provide thorough discharge services to our patients. This is a great opportunity to provide critical support to patients and help set them up for success as they discharge from our facility. Our 330 -bed comprehensive Behavioral Healthcare Facility is located on the northside of beautiful San Antonio! Our facility provides a nurturing, campus-style environment located on 29 beautiful acres and features experiential options such as a swimming pool, gym, ropes course and more.
The person selected for this position will be compassionate and patient focused on patients and mental health. Key responsibilities include:
• Facilitates timely referrals to alternate levels of care and assists family and/or guardian with completion of applications.
• Communicates with referral providers about new referrals and ensures that the facility has all necessary information to consider a referral; assists with ensuring that all receiving providers have all necessary clinical materials and information.
• Documents case management contacts in progress notes, communicates with therapists/treatment team about contact and updates on the status of discharge planning.
• Works with involved agencies by arranging and participating in multi-agency meetings as needed to facilitate a smooth discharge.
• Works with Utilization Review to establish prior authorization, travel, insurance application, etc. is coordinated for a timely discharge.
• Maintains a shared file where information is updated to review status of faxes, referrals, etc; maintains contact with current providers to update them on the status of patient/course of hospitalization and to obtain information from provider to share with treatment team.
• Participates in problem-solving regarding abrupt changes in discharge, coordinates and communicates the discharge plan.
Qualifications
Excellent verbal and written skills. Ability to maintain confidentiality. Computer skills and knowledge of basic case management/utilization review principles.
EDUCATION: Associates Degree or higher in a related field or 5 or more years of experience in mental health field.
LICENSURE/CERTIFICATION: not required
EXPERIENCE: Two years experience in mental health setting and previous experience in Case Management/Utilization Review.
