Qualifications
Your role & responsibilities:
- Communicate with referral sources and parents to discuss the discharge process and to manage discharge plans.
- Identify needs of referrals sources and/or professionals during the period of treatment.
- Oversee the tasks of ongoing communication regarding treatment progress, discharge plans, and time of discharge to outside interested parties.
- Facilitate the transfer of required documents to outside agents.
- Responsible for discharge plans including scheduling of appointments and communication with parents and referral sources.
- Document findings, discharge plans, and actions taken on medical record according to departmental guidelines; prepare reports and maintain records as required.
Qualifications
- High School Diploma or GED required; bachelor's degree preferred
- Must be at least 21 years of age to apply
- Experience in discharge planning or case management
