Arrowhead Behavioral Health is an 48-bed facility, that provides mental health, substance abuse and detoxification for both inpatient and outpatient services in Maumee OH.
Arrowhead Behavioral Health is seeking an organized Discharge Planner in our inpatient setting. In this position, you will be responsible for coordinating aftercare and providing support to patients throughout their discharge process. Candidate must have an understanding of medical terminology and knowledge of community resources and referrals to successful place our patients.

Qualifications:
- Bachelor’s Degree required in Social Work or related field
- Licensed Social Worker (LSW) prefered
- Clerical and organizational experience is required
- Medical terminology and background is preferred
- Excellent written and oral communication skills, time management, and social skills
