What are the responsibilities and job description for the INTAKE COORDINATOR - RN PRN position at UHS?
SUMMARY STATEMENT & PURPOSE
The Intake RN is responsible for responding to admission referrals and other requests for services and information. The Intake RN provides general support to UR and Social Services under the direction of the Lead Clinician and the Director of Admissions. The Intake RN has additional responsibilities in the medical screening of potential patients.

MINIMUM QUALIFICATIONS
Degree in nursing from an accredited college or university.
A minimum of two (2) years of experience as an RN
Washington State RN license or Compact License eligible to work in WA State
Mental Health Professional designation
BLS, Handle with Care, and Verbal De-Escalation certifications will be provided by INBH
PREFERRED QUALIFICATIONS
BSN
Experience in working with programs and outside referrals
Experience working in a mental health setting.

- Receive referral calls, obtain and document demographic and presenting problem information, schedule assessment appointments with Psychiatric Assessment Team (P.A.T.) in a timely manner.
- Complete psychiatric assessments as required.
- Process all requests for information and external referrals.
- Provide customary and requested information to referral sources.
- Assist with arranging for the admission of patients, including notifying the business office, verifying benefit coverage, initiating calls to physicians/licensed practitioners as required, entering necessary information into computer tracking system, notifying program staff, and distributing records to all necessary staff.
- Assist in determining what releases of information are required and process releases according to policy.
- Assist in obtaining the appropriate patient documents and records.
- Maintain assessment and referral statistics and other data as assigned, including P.A.T. log, pending admissions file, HMS statistics and reports.
- Initiate “Initial Call Sheet form of all incoming referral calls, document disposition and other appropriate information.
- Establish an effective relationship with customers, ensuring that appropriate customer service is provided.
- Understand and apply the established medical clearance criteria when screening patients for admission.
- Review all available medical information, foundational lab work, medical work ups to determine the medical stability of potential patients. Perform any RN to RN consultations as needed to ascertain medical stability of patients prior to transfer.
- Provide initial medical screening for patients who present as unscheduled assessments at the hospital (including obtaining vital signs, UDS when indicated). Liaison and consult with Fairfax medical staff regarding any emergent medical conditions.
- Take and transcribe admission orders as needed from admitting medical staff.
