Responsibilities
Fairmount Behavioral Health Hospital provides comprehensive behavioral healthcare for children, adolescents, adults, and senior adults suffering from mental health and addictive illnesses. Fairmount is a 239 bed, short-term, acute care hospital offering inpatient, partial and intensive outpatient hospitalization, as well as specialized treatment programs for chemical dependency. The Health Information Services Clerk/Coder participates as an integral member of the records management team by ensuring the quality maintenance of patient information/medical records, within all laws, rules and regulations of federal and state licensing agencies, and JCAHO standards for the quality of patient care.
- Perform coding and abstracting functions daily with the medical record utilizing IC9 and ICD10.
- Acts as the liaison between the contracted transcription service and hospital.
- Print transcribed reports on a daily basis and log in transcription log.
- Audit transcribed reports for accuracy and document findings.
- Update deficiencies in HMS Deficiency Tracking system and print reports on a regular basis and as requested.
- Provide guidance to physicians/licensed practitioners and facility personnel in completion of medical records, in compliance with all department and hospital policies and procedures.
- Answer inquiries from internal and external contacts regarding information recorded in the patient’s medical records, within all departmental policies and guidelines.
- Notify the Supervisor regarding any potential problems with records updates, maintenance and/or breaches of confidentiality.
- Perform other duties as assigned by Supervisor.
- Verify discharges daily and review the Billing Readiness Report.

Qualifications
Requirements
Education: Graduation from an accredited medical records program preferred.
Experience: At least three (3) years experience in health care information management/medical records, thorough knowledge of medical terminology preferred.
Additional Requirements: Knowledgeable coding/reimbursement procedure. Knowledgeable medical records and computer application. Clinical chart completion methodology and knowledgeable of legal aspects and quality assurance. Effective oral and written communication skills. May be required to work occasional overtime and flexible hours.
