The Brook Hospital Dupont located at 1405 Browns Lane is currently seeking a Time Team Mobile Assessor  to join our team.

A Masters Degree is required for this position.

Qualifications

Job Duties/Responsibilities:

• Conducts initial evaluation to determine an appropriate treatment level of care at the facility, via telehealth, or out in the field
• Answer incoming intake calls and provide appropriate resolution.
• Responsible for completing admission paperwork, including admission data entry.

• Gathers all information pertaining to patient’s physical, psychological, and social status and documents clinical information received from patient, guardian, referral source, etc. to make accurate level of care determination
• Discusses evaluation information and recommendation with treating physician
• Works with intake department and other hospital departments
• Conducts insurance authorization for patients admitted to any Brook Hospital program
• Position requires person to work with minimal supervision
• Best candidates for this position will have behavioral health experience, a strong work-ethic, and highly consistent timeliness and attendance!

Qualifications

 EDUCATION:
Required: Master's Degree from an accredited school in the Behavioral Sciences

LICENSURE / CERTIFICATION:
Preferred: licensed or under supervision in state of Kentucky (CSW/LCSW CADC/LCADC, MFTA, LMFT, LPCA, LPCC, LPA, LPP)
