Providers with South Texas Health System (STHS) Clinics deliver high quality family medicine, health screenings and advanced specialized care to the people of the Rio Grande Valley. Independence Physician Management (IPM), a subsidiary of UHS, was formed in 2012 as the physician services unit of UHS. IPM develops and manages multi-specialty physician networks and urgent care clinics which align with UHS acute care facilities. It also provides select services for the Behavioral Health division of UHS. Through continuing growth, IPM operates in 11 markets across six states and the District of Columbia. Our leadership team, practitioners, and teams of healthcare professionals are collectively dedicated to improving the health and wellness of people in the communities we serve.

Position Summary:
 

The Clinician will be responsible for working with patients and their families.  This will include social assessments, individual / family / marital / counseling, group therapy, patient/family education, discharge planning and other modalities as applicable.

• Completes psychosocial assessment as per policy on patients and reassesses as necessary for all patient populations and makes recommendations to the case manager and to the multidisciplinary team.
• Collaborates with the multidisciplinary team to develop and individualize discharge plan for each patient.
• Facilitates multi-disciplinary consultation and collaboration as needed with physicians, nurses, nutritionist, case manager, psychiatrist and pharmacist and other members of the treatment team as needed.
• Demonstrates understanding of legal issues/regulations related to factual competency, guardianship for age group served.
• Participates in multidisciplinary discharge planning discussions, and offers input or reports open cases.

Qualifications

Education

• Master's Degree in Social Work or Master’s Degree in Counseling, Education, or Related field. 

Licensure & Certification

• Must be licensed through the State of Texas (LMSW; LCSW; LPC; LPA; LPCI)
• Hold a current BLS.

Experience

• Minimum of 3 years work experience.
• Experience in healthcare or psychiatric setting preferred, Bilingual preferred. 

Essential Skills 

• Must demonstrate commitment and adherence to STHS Clinics Compliance Program and Code of Conduct through compliance with all policies and procedures, the Code of Conduct, attendance at required training and immediately reporting suspected compliance issue(s) to the Compliance Officer.

 Working Conditions – Combination of medical office and exam/procedures room settings.  Well ventilated adequate space.clinical experience preferred
