You will be responsible for managing all aspects of patient care from the initial assessment to ongoing support, while also overseeing and coordinating the activities of our caregiving staff.
Responsibilities:
- **Patient Assessment and Care Planning:
- Conduct comprehensive assessments of patients' needs, preferences, and goals for care.
- Develop personalized care plans in collaboration with patients, their families, and healthcare professionals.
- Regularly review and update care plans to ensure they accurately reflect the evolving needs of patients.
- **Coordination of Care:
- Serve as the primary point of contact for patients and their families, providing support, guidance, and advocacy throughout the care process.
- Coordinate all aspects of patient care, including scheduling appointments, arranging transportation, and coordinating with other healthcare providers.
- Ensure timely communication between patients, caregivers, and healthcare team members to facilitate continuity of care.
- **Staff Management and Support:
- Recruit, train, and supervise caregiving staff, ensuring they have the necessary skills and resources to provide high-quality care.
- Assign caregivers to patients based on their needs and preferences, and maintain appropriate staffing levels to meet demand.

Qualifications:
- Proven experience in care coordination or case management, preferably in a home care setting.
- Strong interpersonal and communication skills, with the ability to build rapport with patients, families, and caregivers.
- Excellent organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities simultaneously.
- Knowledge of relevant healthcare regulations and best practices in home care.
