Overview:

Unacare Health Care Agency is seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a pivotal role in ensuring the seamless coordination of care for our patients and effective communication between patients, their families, and our staff. You will be responsible for managing all aspects of patient care from the initial assessment to ongoing support, while also overseeing and coordinating the activities of our caregiving staff.

Responsibilities:

  • **Patient Assessment and Care Planning:
  • Conduct comprehensive assessments of patients' needs, preferences, and goals for care.
  • Develop personalized care plans in collaboration with patients, their families, and healthcare professionals.
  • Regularly review and update care plans to ensure they accurately reflect the evolving needs of patients.
  • **Coordination of Care:
  • Serve as the primary point of contact for patients and their families, providing support, guidance, and advocacy throughout the care process.
  • Coordinate all aspects of patient care, including scheduling appointments, arranging transportation, and coordinating with other healthcare providers.
  • Ensure timely communication between patients, caregivers, and healthcare team members to facilitate continuity of care.
  • **Staff Management and Support:
  • Recruit, train, and supervise caregiving staff, ensuring they have the necessary skills and resources to provide high-quality care.
  • Assign caregivers to patients based on their needs and preferences, and maintain appropriate staffing levels to meet demand.
  • Provide ongoing support and supervision to caregivers, addressing any concerns or issues that may arise during the course of care.
  • **Quality Assurance:
  • Monitor the quality of care provided to patients, conducting regular assessments and audits to identify areas for improvement.
  • Implement quality improvement initiatives to enhance the delivery of care and ensure compliance with regulatory standards.
  • Respond promptly to any complaints or grievances from patients or their families, taking appropriate action to address their concerns.
  • **Documentation and Reporting:
  • Maintain accurate and up-to-date records of patient assessments, care plans, and interventions.
  • Generate reports on key performance indicators, such as patient satisfaction, caregiver productivity, and adherence to care plans.
  • Ensure compliance with all documentation requirements and privacy regulations.

Qualifications:

  • Proven experience in care coordination or case management, preferably in a home care setting.
  • Strong interpersonal and communication skills, with the ability to build rapport with patients, families, and caregivers.
  • Excellent organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities simultaneously.
  • Knowledge of relevant healthcare regulations and best practices in home care.
  • Proficiency in computer applications, including electronic health record systems and Microsoft Office Suite.
  • Compassionate and empathetic approach to patient care, with a commitment to providing the highest level of service.
