POSITION SUMMARY
Responsible and accountable for coordinating the care of patients meeting case management criteria across a continuum of care. Ensures and facilitates the achievement of quality, clinical and cost outcomes. Negotiates, procures and coordinates services and resources needed by the patient and family.
EDUCATION, CREDENTIALS, TRAINING and EXPERIENCE:
Minimum Required:
Care Management Specialist: Graduate of an accredited school of professional nursing, with current license to practice as a Registered Nurse in North Carolina. Thorough knowledge of nursing care process from admission through discharge, gained through several years of clinical experience as a RN
Psychiatric Care Management Specialist: Graduate of an accredited school of professional nursing, with current license to practice as a Registered Nurse in North Carolina or BSW/MSW from an accredited institution. Thorough knowledge of processes from admission through discharge, gained through several years of clinical experience.
Preferred:
Care Management Specialist: Baccalaureate Degree in Nursing and previous experience as a Care Management Specialist. Knowledge of utilization review and discharge planning methods and procedures. Understanding of clinical pathways and disease management models.
Psychiatric Care Manager: MSW certified as a Clinical Social Worker
