Plan of care which includes goals and prioritized nursing/medical approaches derived from data collected. Utilizes additional resources in goal setting and meeting learning needs. Implements strategies to provide for patient#s participation in goal setting, health maintenance and maximization of health capabilities. Notifies physician of significant changes in patient#s condition and potential or suspected complications. Evaluates the patient for diabetes knowledge and status of diabetes management. Qualifications # Licenses: Currently licensed by the Indiana State Board of Nurses# Registration and Nursing Education as a registered nurse A minimum of one year in diabetes education preferred Certification by the American Association of Diabetes Educators (CDE) required or willing to obtain within 2 years of hire.
