The Division of Prevention Services partnered with UAU and Staten Island schools to offer families flexible service options and support. This is part of the broader interagency efforts between ACS and the DOE to prevent families at risk of receiving ACS interventions at home.

POSITION SUMMARY: The Supervisor for the School Based Early Support program will be under the direct supervision of the Director of Prevention Family Services. The Supervisor is responsible for conducting themselves in a professional and appropriate manner, setting examples to the youth and parents in which the Supervisor is providing services to, mentoring, and managing staff. The Supervisor will be responsible for the management of staff and case management of approximately 80 families. The Supervisor will be expected to effectively communicate, provide appropriate documentation as outlined below and adhere to deadlines as set forth by the Director of Prevention Family Services.

PRIMARY JOB RESPONSIBILITIES (include but are not limited to):

· Responsible for management and review of an average case load of 80 families. At times, at the direction of the Director of Prevention Family Services or Quality Assurance Program Manager the Supervisor will be responsible for directly manage their own case management.

· Properly and effectively review case progress notes, outstanding items to complete within CONNECTS, conference notes and assessments for content and accuracy.

· Monthly review of all case notes to ensure completion of ongoing assessments, required contacts with all tracked children and documented progress towards service goals. As required, to also include addressing and identifying risk factors.

· Attend offsite meetings such as in-home visits, school visits, mediation meetings etc. and as directed by the Director of Prevention Family Services.

· Provide, schedule, and conduct direct supervision, once a week, for a minimum of one hour, per Case Planner (6), and per case aide (2). Complete and provide applicable proper documentation from direct supervision and record accordingly based on department protocols, procedures, and policies.

· Ensure all case file documentation is provided to the Quality Assurance Program Manager on a weekly basis.

· Monitor staff field schedules, home visit schedules, and school site schedules.

· Facilitate Triple P Parenting classes 4 times per year.

· Co-design strategy and planning for (4) school-based offerings per year with DOE staff and the School Outreach Liaison.

· Responsible for family outreach and engagement efforts at designated school sites (P.S 44, I.S 2, and I.S 34) and the community

· Responsible for appropriately logging supervision on the designated form and designated location accordingly. Deadline to provide monthly notes must be completed by the 30th of each month.

· Responsible for daily review, corrections, and turn in final FASP reports to Director of Preventive Family Services, two days prior to Connections due date.

· Continuously review Connections notes to ensure case planners are entering case contact notes within the standard time frame, 3 days from event date.

· Ensure and monitor track of staff’s training hours (46) per fiscal year.

· Review all assessments and ensure they are completed within 45 days of service acceptance.

· Schedule monthly and as needed one on one meetings both collectively and individually with Director of Family Preventive Services and counterpart ACS Supervisor, to review projects, cases, deadlines, issues, concerns, workflows, etc.

· Conduct case reviews once per month for each case and document in Connections by the 15th of the following month.

· Provide guidance, support and cooperation related to department audits.

· Compete required ACS training (42 hours per year), continuing education conferences and department related conferences as approved and directed by the Director of Prevention Family Services.

· Properly and accurately follow protocols, related to case practice management and consultation; specific practice models include but not limited to Family Support Programs, Mobility Mentoring and Solution-Based Casework.

· Other duties and responsibilities as assigned.

JOB QUALIFICATIONS:

  • Master’s degree or equivalent graduate degree in Human Services required
  • Bilingual required
  • Valid and current LMSW, LMHC, CASAC, or CASAC-T - preferred
  • 3 years or more of case management experience
  • 2 years minimum of relevant child welfare experience

· 1 year minimum of staff supervisory experience

· Obtain and maintain all security clearance and training requirements throughout the duration of employment.

· Proficient with CONNECTIONS and PROMIS (ACS data systems), preferred

· Must be culturally competent - the ability to understand, be sensitive to the needs of, communicate with, and effectively interact with people across all cultures.

· Familiar with the regulations and requirements for ACS and NY State Departments of Health and aware of the compliance requirements as enacted by Federal, State and local jurisdictions as applicable based on the department’s contractual obligations.

· Ability to demonstrate professional excellent employee rapport and community leadership.

· Proficient writing and verbal communication skills.

· Proficiency in basic computer usage, i.e., Windows, Microsoft Suite, Google Apps (drive, sheets, docs) etc.

· Excellent organizational and time management skills.

· Excels working in a collaborative environment to achieve target objects and outcomes.

· Incumbent will be scheduled based on operational need.

· Standard office equipment including computers, fax machines, copiers, printers, telephones, etc.

· Position is in a well-lighted office environment.

· Involves sitting approximately 80% of the day, walking or standing the remainder.

· May include travel to a variety of locations, based on programming needs. Which is reimbursed in accordance with UAU’s fiscal policies and procedures. Also requires a reliable mode of transportation.

· May include working prolonged periods of time sitting at a desk and working on a computer.

· The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.

· The ability to observe details at close range (within a few feet or more of the observer).

Job Type: Full-time

Pay: $60,000.00 - $68,000.00 per year
