In the Health and Social Services Manager role, you will lead a team of Health and Social Services Coordinators, while overseeing day-to-day clinical operations, in the state of New Jersey, and participating in business development activities. You will manage a program or region, including team and performance management, process improvement, and standardized execution of clinical decision processes, and implementation of new program initiatives. Also complying with contractual requirements and ensuring member needs are met.

Primary Responsibilities:
- Selects, manages, develops, mentors, and supports staff in designated department or region
- Develops clear goals and objectives for performance management and effectively communicates expectations and holds the team accountable for results
- Engaged in performance improvement initiatives to optimize clinical workflows, promote team engagement, and continuously improve care management of health plan members
- To meet the unique needs of our members, have an intimate understanding of the contractual requirements
- Identify, select, structure, and prioritize projects, ultimately implementing changes to meet program requirements
- Ensures standardized execution of workflow processes, including conducting performance audits, quality reviews, and compliance adherence
- Manages the day-to-day operations for community-based, nursing facility, and assisted living care managers in surrounding counties, which included resolving problems, promoting collaboration to ensure the team meets the established performance metrics and organizational goals
- Oversee interdisciplinary teams for Nursing Facility transitions
- Collaborate with Provider Relations team to assist in resolution of issues presented by providers
- Conducts staff training and coaching of direct reports as needed
- Conducts bi-annual field visits with direct reports to observe, provide areas of teaching, address issues and concerns and foster a good working relationship
- Collaborates across Optum and UHG and interacted with Medical Directors, Site Directors, Senior Leaders, Network, Marketing, Account Management, Quality, Product, and other stakeholders
- Serves as a direct liaison for members, nursing facility providers and assisted living providers and nursing facility transition programs (I Choose home New Jersey)
- Review and manages operational reports to monitor staff productivity and program effectiveness
- Other duties as assigned
- Traveling up to 20% to NJ for meetings, conferences, etc

Required Qualifications:
- Unrestricted RN license in the state of New Jersey, or Licensed Clinical Social Worker, or bachelor’s degree in clinical or behavioral field
- 5 years of experience working within the community health setting in a health care role
- Experience coaching or mentoring staff
- Intermediate level of proficiency with Microsoft Office suite (Word, Excel, SharePoint, PowerPoint, etc.), with the ability to navigate a Windows environment
- Travel up to 20 percent for meetings, conferences, etc

Preferred Qualifications:
- Master’s degree
- NJ Choice Master Trainer Certification
- 3 years of case management leadership experience within a healthcare industry
- Case Management experience
- Experience/exposure with discharge planning
- Experience in utilization review, concurrent review and/or risk management
- Background in managed care
- Bilingual in English/Spanish

Colorado, Connecticut or Nevada Residents Only: The salary/hourly range for Colorado residents is $82,100 to $146,900. The salary/hourly range for Connecticut/Nevada residents is $90,500 to $161,600. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
