LVN Case Manager

Primary Responsibilities:

• The LVN Case manager will perform clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating). This includes case management, coordination of care and medical management consulting
• Provide health education, coaching and treatment decision support for members

Required Qualifications:

• High school degree or GED or higher
• Current unrestricted Texas LVN license or compact license
• Current BLS and/or CPR certification or the ability to obtain within 30 days
• 2 years of experience in a physician’s office, clinical or hospital setting
• Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
• Proficient computer skills to work efficiently with electronic medical records
• This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor’s diagnosis of disease
• Full COVID-19 vaccination is an essential requirement of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance

Preferred Qualifications:

• Experience related to patient education and/or motivational interviewing skills and self-management goal setting
• Case management experience
• Bilingual (Spanish)
• Solid interpersonal skills
• Ability to interact productively with individuals and with multidisciplinary teams
• Excellent organizational and prioritization skills
