Job Description

Job Summary: Provide nursing and customer service support to facility staff in long term care facilities through education programs, support and pharmacy coordination

Key Responsibilities:

• Maintain and update medical records such as treatment plans and physician order sheets at long-term care facilities
• Conduct medication storage surveys. Checking the medication storage facilities and the medication carts for proper storage of medications, cleanliness and removal of expired medications. Submitting a written report and recommendations for each review of medication storage
• Conduct Medication Pass reviews. Assessing the performance of the nursing staff in medication administration through the process of medication pass observation, as requested by nursing and administration, and as necessary.
• Entrance and exit interviews with facility’s Director of Nursing (DON) and staff concerning nursing and pharmacy services
• Assist DON and staff with troubleshooting and correcting problem areas and/or improving pharmacy services
• Work closely with consultant pharmacist to provide interdisciplinary approach to patient, consulting and specialty services as required
• Conduct mock surveys as requested and scheduled via the pharmacy to ensure compliance with the Department of Health surveys
• Develop and prepare material and in-service education for facility staff
• Involved in customer service and marketing services provided by the pharmacy
• Emar reconciliations
• Cycle medications reconciliation
• Manage resolution of client issues and requests
• Checking the emergency medication supply to ascertain that it is properly sealed and stored and that the contents are not outdated.
• Other duties as assigned; Job duties may vary by location

Job Types: Full-time, Contract

Pay: From $30.00 /hr -Based on experience
