Description:

The Program Services Specialist provides community education and critical life resources to the Illinois Department of Corrections (IDOC) Telemedicine patients, specifically in Diabetes care, during incarceration and re-entry. Build collaborations and linkages with partnering agencies that help ensure a smooth transition for the IDOC population. Maintain programmatic data sets kept for approved clinical and research activities.

Duties & Responsibilities:

  • Assist with the Diabetes Management Clinic by developing greater education on the disease process and support with scheduling/referring to the UI Health Diabetes Management clinics post-release, identifying resources in the community relating to possible insurance, other medical resources, and other ancillary services for pending and post-release patients.
  • Enhance capacity building statewide regarding identifying employment and low-income housing opportunities for our re-entry patients.
  • Manage CADE and additional data bases and sets to ensure that information is available for statistical reporting.
  • Create linkages with community care service providers throughout the state and make provisions to travel in the immediate future to discuss streamlining referral processes for transitioning telemedicine patients.
  • Participate in divisional and departmental research activities related to incarcerated and re-entry populations.
  • Performs other related duties and participates in special projects as assigned.


  • Minimum Qualifications Required:


    1. Any one or combination totaling three (3) years (36 months) from the categories below:

    A. Coursework in social work or behavioral sciences, or closely related field as measured by the following conversion table or its proportional equivalent:
    • 30 semester hours equals one (1) year (12 months)
    • Associate's Degree (60 semester hours) equals eighteen months (18 months)
    • 90 semester hours equals two (2) years (24 months)
    • Bachelor's Degree (120 semester hours) equals three (3) years (36 months)

    B. Progressively more responsible work experience in social service/ counseling at a paraprofessional level under qualified supervision.

    2. Minimum of one year of experience, including volunteer engagement with individuals receiving post-release (re-entry) services, such as facilitating access to healthcare, medication assistance, and community and mental health support.
